I hereby give my permission for my child(ren) to participate in Warrior Wrestling. Participation in any program which involves physical activity exposes the member to certain risks and dangers. Accidents and injuries are always a possibility, and it is impossible to foresee and protect the member from all conceivable dangers. I hereby affirm that my child(ren) has/have no conditions that would make it unsafe for him/her/them to participate in the camps activities selected. Medical Consent: I understand that the coaches will make every effort to contact me in the case of an emergency. I give my permission for the coaches to administer any medication needed and to provide and arrange for and consent to any necessary medical treatment for my child(ren) while at the Warrior Wrestling building/offsite, including onsite and offsite emergency care. I accept responsibility for the costs of all such medical treatment. By signing below, I am promising that my child has health insurance and I will assume any costs related to his health or injuries. I understand that monthly dues are to be kept current and failure to do so means that my child must pay the drop-in fee for each practice. I understand that all payments are drafted automatically each month and refunds are not given if already drafted for that month. It is not Warrior Wrestling’s Responsibility to cancel memberships if they are not used and it is the clients responsibility to keep track of their finances and recurring monthly subscriptions. If membership lapses due to non-payment, I am responsible for any monies paid up to that point and should expect no refund of monies paid towards future endevours with Warrior Wrestling. The member will also be invoiced for any practice they attend after the lapse in their membership at the current advertised rate. Cancellations can be done via Paypal. There is a cancellation fee per member of $10 to be paid for each cancellation. If you choose to re-up your membership after cancellation, you will pay the most recent price (current membership rate) for membership. Scholarships are reviewed on an individual basis and you must fill out the “Member’s Hardship Request” before delinquency. Please reach out to 717-521-9896 if you are experiencing a hardship. We do not want the kids to be affected by family money issues and will do our best to assure that their lives remain as close to normal as possible while you get your finances in line. We reserve the right to refuse service to anyone for any reason and past delinquencies will be taken into consideration. We expect all coaches and parents to be treated with respect by members and family members. We treat all behavioral challenges as they arise but will not tolerate bullying or blatant disrespect towards anyone. Failure to act in a respectful manner could result in disciplinary action or removal from the club. We do everything possible to avoid this and will work with the children to learn their boundaries. In consideration of child(ren)’s participation at Warrior Wrestling, and without any further consideration from Warrior Wrestling, I hereby grant permission to the Coach and affiliates to utilize my child(ren)’s appearance, performance, or voice in any and all manner and media throughout the community for the purpose of promotion, reporting or publication. The Coach/owner may use my child(ren)’s, likeness and voice in connection with publication, promotion, exhibition and distribution of such material. I understand that no royalty, fee or any other compensation of any kind shall become payable to me by reason of such release and use of any photograph.
